Fee Schedule

• Initial Consultation:  Free.
• Initial Intake Assessment Fee:   $100.
  (This is a one-time charge.)
• Monthly Medical Management Fee:  $250.
  This includes initial lab tests, initial and subsequent monthly medical management.  This is due every month.
• Counseling Charges: Counseling is included up to 2 sessions per month with no additional charge.
• Additional Counseling Charges: $50 per session. To be determined by MD and counselor. This to be prepaid with monthly Medical Management fee.
• Group Sessions: $25 each.
• Pharmacy Charges: $ Depends on Rx drugs prescribed; Select Rx cards are accepted.
• Additional Lab Test Charges: To be determined by MD, if needed.
